Site Induction Declaration

SAIYU Site Induction – Declaration and Acknowledgement

By signing this declaration, I confirm and acknowledge that:

  • I hold a current General Construction Induction (White Card) and will carry evidence while on site.
  • I have completed the SAIYU site-specific induction and understand the hazards, controls, and rules that apply to this site.
  • I have been briefed on emergency response and evacuation procedures, the location of first aid facilities/equipment, and muster points.
  • I understand how to report hazards, near misses, and incidents immediately to my supervisor or site management, and that certain incidents may be notifiable to the regulator.
  • I will follow the Work Health and Safety (WHS) Act and Regulations, relevant Codes of Practice, site rules, SWMS and permits, SDS, and posted signage.
  • I will only perform work for which I am trained, competent, and authorised, and I will use the required PPE as instructed.
  • I will not work while impaired or under the influence of alcohol or drugs and consent to testing if required by company policy.
  • I will stop work and promptly report if I consider a task unsafe or I do not understand the required controls.
  • If applicable, I have provided (or will provide) evidence of licences/competencies and required insurances to SAIYU or the principal contractor and will present them upon request.
  • I consent to SAIYU collecting and securely storing my induction records (including digital signature, White Card and licence details) for compliance and audit purposes and to share them with regulators or emergency services where required by law.
  • The information I provide is true and correct. I understand that providing false or misleading information or failing to follow safety requirements may result in removal from site.

  • 我持有有效的建筑业通用入职卡（White Card/白卡），并在现场随身携带证明。
  • 我已完成赛宇（SAIYU）现场入职培训，理解适用于本工地的危险、控制措施与现场规则。
  • 我已了解紧急响应与疏散程序、急救设施/设备与集合点的位置。
  • 我理解隐患、未遂事件与事故的即时报告流程，并知悉部分事件可能需要依法向监管机构通报。
  • 我将遵守《工作健康与安全（WHS）法与条例》、相关实务守则、现场规则、SWMS 与许可、SDS 及现场标识。
  • 我仅从事本人已培训、胜任且获授权的工作，并按要求正确使用个人防护装备（PPE）。
  • 我不会在受酒精或药物影响、或能力受损的情况下工作，并同意在公司政策要求时接受检测。
  • 如认为作业不安全或不清楚控制措施，我将立即停工并上报。
  • 如适用，我已向赛宇或总承包方提供（或将提供）相关资质/执照与必需保险，并在要求时出示。
  • 我同意赛宇为合规与审计目的收集并安全保存我的入职记录（包括数字签名、白卡与执照信息），并在法律要求时与监管或应急机构共享。
  • 我确认所填信息真实、准确；如提供虚假/误导信息或不遵守安全要求，可能被要求离场。
